Dear Parents & Legal Guardians,

Thank you for your continued patience and trust as we work together in making the Oakridge Campus both safe and enjoyable for all. The health and safety of your student(s) and our community is our top priority. With the evolving situation around COVID-19, we want to make sure you and your student know the best ways to protect yourselves and others when on campus. The goal of this form is to ask for your partnership in acknowledging that you have received and read the Campus Overview document, and for you to acknowledge the current environment, including your student's return to campus.


As with the transmission of any communicable disease like a cold or flu, you may be exposed to COVID-19, also known as “coronavirus,” at any time or in any place. The guidance document that you have received is in place to enhance campus safety and reduce the impact of COVID-19 conditions upon returning to campus. Be assured that we will continue to monitor guidance from the Centers for Disease Control and Prevention (CDC), World Health Organization (WHO), the American Academy of Pediatrics (AAP), and state and local government health agencies to limit the transmission of COVID-19, while respecting the legal rights of parents and their children.


However, even with careful attention to recommended precautions, there is still a chance that you or your child could be exposed to an illness while at Oakridge Private School, just as you might be at your place of work, grocery stores, restaurants, or local businesses. Additionally, although significant measures are being taken to promote physical distancing and multiple health and safety standards, it may not always be possible to maintain physical distancing between students, staff, and parents. In short, there are no guarantees that your student will not be exposed while on campus.


The decision to be enrolled at Oakridge Private School is voluntary and may include possible exposure to, and illness from communicable diseases, including but not limited to a common cold, influenza, COVID-19, and other communicable diseases. While physical distancing and personal protection equipment may reduce this risk, there are unique health risks due to COVID-19, including serious illness and death, especially to those with underlying health conditions. All Oakridge Private School safety requirements or protocols, for your student’s presence on campus, must be followed by your student.
